Please note that, with the exception of mammogram bookings, all bookings require a referral letter from a qualified doctor. In order to make an online booking, your referral letter will need to be uploaded to this page. If you cannot scan your referral letter, you may simply take a photo of it and upload it in this manner. Please ensure that your document is clear and readable. All your information will be kept confidential and cannot be accessed by anyone else using this website.

NB: Please remember to attach your Medical Aid card and I.D. with your referral form and also to bring with you on the day of your appointment.
